Community Support - Supporting Your Growth

Our Purpose

Our mission and purpose is to provide you support while empowering you to grow and become independent in the community. We will assist you in finding resources, filling out paperwork, and removing unnecessary barriers to help you be successful.

Who We Serve

We serve those 19 years and older with substance use disorders and/or those with severe and persistent mental illnesses.

 

Our service area includes all 22 counties of Region 4 Mental Health: Antelope, Boone, Boyd, Brown, Burt, Cedar, Cherry, Colfax, Cuming, Dakota, Dixon, Holt, Keya Paha, Knox, Madison, Nance, Pierce, Platte, Rock, Stanton, Thurston and Wayne.

Qualifications

Mental Health

   ~ CSP-MH Admission Requires:

  • Successful Referral.

  • Completion of in-house mental health assessment (IDI) at BHS

  • Qualifying Diagnoses Include: Major Depressive Disorder, Schizophrenia, Chronic PTSD, Bipolar I & II Disorder, Schizoaffective Disorder

  • Other diagnoses may qualify on a case-by-case basis. 

Substance Use

   ~ CSP-SA Admission Requires:

  • Successful Referral

  • Completion of a Substance Use Evaluation by an agency

  • Less than one year of clean time

  • Qualifying Diagnoses include: Moderate substance Use Disorder, Severe Substance Use Disorder

Referral

You may refer yourself to Community Supports or you may be referred by your therapist, doctor, probation officer, etc.

The Connection Project

Join Community Support every Wednesday at 1:30 PM at the Connection Project for fun games and activities. The Connection Project is a safe and positive place to find peer support and mutual relationships. All are welcome. The Connection Projection is located at 204 W.
